Dermal Fillers guide

What are dermal fillers?

Dermal fillers are substances injected into areas of the skin with volume loss, lines or shape differences. The aim is to restore lost volume, soften folds or define the contours of an area. The procedure is not surgery. No incision is made and general anaesthesia is not needed.

Fillers are used on the lips, chin, cheeks, under-eye hollows, nose and, less often, the hands and other body areas.

What types of filler are there?

Fillers are grouped as temporary, long-acting or permanent, depending on how long they stay in the body. Hyaluronic acid is the substance used most often on the face. It is a molecule found naturally in the skin that holds water. In filler products it is cross-linked into a gel, which helps it last longer in the body.

FillerStructureDuration (usually)Good to know
Hyaluronic acidGel form of a molecule found naturally in the body6 to 18 months, depending on product and areaCan be dissolved with hyaluronidase
Calcium hydroxylapatiteMicroscopic mineral particles in a gelAbout 12 to 18 monthsCannot be dissolved; not advised for lips
Poly-L-lactic acidBiodegradable synthetic that stimulates collagenBuilds over months, can last up to 2 yearsNeeds several sessions
Fat injectionYour own fat tissueThe part that takes can last long; absorption variesRequires liposuction
Permanent syntheticsNon-degradable substances such as PMMA or liquid siliconePermanentHardness, lumps and granulomas can appear years later; hard to remove

The main advantage of hyaluronic acid is that a doctor can dissolve it with hyaluronidase if the result is unwanted or a complication develops.

What is the difference between fillers and Botox?

Fillers and Botox are two separate treatments for different problems. Filler replaces missing volume, while Botox temporarily relaxes a muscle. Botox is a medicine containing botulinum toxin. It is used for lines caused by facial expressions, such as forehead lines, frown lines and crow's feet. Its effect usually lasts 3 to 6 months.

Filler suits volume loss, deep folds and lines visible when the face is at rest. The two can be planned together for different areas of the same face.

How are dermal fillers injected?

A filler treatment starts with an examination and is usually completed in the same visit. It generally takes 15 to 45 minutes, depending on the area.

  1. Examination and plan: The doctor assesses your face and asks about your health history, medicines and previous filler or Botox.
  2. Information: The product, expected result and possible risks are explained. Written consent is taken, and most clinics take photos before treatment.
  3. Preparation: The skin is cleaned with antiseptic, and numbing cream is applied if needed. Many hyaluronic acid products contain the local anaesthetic lidocaine.
  4. Injection: Filler is placed at the planned depth with a fine needle or a blunt cannula, adding small amounts while checking symmetry.
  5. Aftercare: Gentle massage or a cold compress may be used. Most doctors schedule a check about two weeks later.

Who is a good candidate for fillers, and who is not?

Fillers can be used in healthy adults with volume loss or a shape difference in a specific area. Realistic expectations are part of being a good candidate. Fillers do not stop ageing and cannot lift sagging skin on their own.

Treatment is usually postponed or not performed in these situations:

  • Pregnancy and breastfeeding
  • Active infection, acne flare or cold sores in the area
  • Known allergy to the filler or to lidocaine
  • A bleeding disorder or use of blood thinners
  • Some autoimmune diseases and uncontrolled chronic illness
  • An earlier filler of unknown content in the same area

The final decision on suitability is made by the doctor after an examination.

How long do dermal fillers last, and what is recovery like?

Hyaluronic acid filler usually lasts 6 to 18 months. It tends to wear off faster in areas that move a lot, such as the lips. Deeper treatments in the chin and cheeks may last longer. Product density, the amount used and your metabolism also affect duration.

Recovery is quick for most people. Redness, swelling and bruising usually ease within a few days, sometimes a week or two. Many people return to work the same day. Doctors usually advise avoiding hard exercise, saunas and alcohol for a day or two, and not pressing on or trying to reshape the filler yourself.

What are the risks and side effects of dermal fillers?

The risks range from common mild effects to rare but serious complications. Knowing them helps you recognise warning signs early.

  • Common effects: Redness, swelling, bruising, tenderness and mild asymmetry, which usually resolve on their own.
  • Less common effects: Infection, cold sore flare, allergic reaction, or palpable firmness and lumps. Filler placed too superficially in thin skin can look bluish, known as the Tyndall effect.
  • Rare but serious complications: If filler enters or presses on a blood vessel, blood flow is blocked. This can cause skin tissue death. If vessels supplying the eye are affected, vision loss can occur.
  • Late problems: Inflammatory lumps called granulomas can form months or years later, especially with permanent substances.

If you have sudden severe pain, skin blanching, discolouration or a change in vision during or after treatment, contact a doctor without waiting. With hyaluronic acid, early treatment can dissolve the filler.

What affects the cost of dermal fillers?

The cost depends on the product and the amount used. Other factors include:

  • The type and brand of filler
  • The number of syringes, measured in millilitres
  • The number of areas treated
  • The doctor's specialty and experience
  • The city where the clinic is located
  • Whether a follow-up or touch-up visit is included

You can see the current price range in the pricing section of this page. The exact fee is set after an examination.

How do you choose a doctor for dermal fillers?

Filler is a medical procedure and should be performed by a doctor. In Turkey it is mostly done by dermatologists, specialists in plastic, reconstructive and aesthetic surgery, and doctors trained in medical aesthetics. You can check:

  • Are the doctor's specialty and filler experience clearly stated?
  • Is the treatment done in a licensed healthcare facility?
  • Is there an examination, and is your health history asked about?
  • Are the product name, box and batch number shown to you?
  • Are risks explained and written consent taken?
  • Is it clear how to reach the doctor if a complication occurs?
  • Does the clinic keep hyaluronidase and other emergency supplies?

You can compare doctors who perform filler treatments on DoktorNo by specialty and city.

The information on this page is for general information only and is not medical advice. The suitable method for you can only be determined by a physician examination.

Dermal Fillers Frequently Asked Questions

Can fillers be dissolved?
Hyaluronic acid fillers can be dissolved with an enzyme called hyaluronidase. It is used both for unwanted results and for emergencies such as a blocked blood vessel. Calcium hydroxylapatite and permanent fillers cannot be dissolved this way.
Will my face swell after filler?
Mild swelling and bruising are common after treatment. They usually ease within a few days, though in some people they last a week or two. If swelling increases sharply or pain gets worse, contact your doctor.
Who should perform dermal filler injections?
Filler is a medical procedure and should be done by a doctor. It is usually performed by dermatologists, plastic surgeons or doctors trained in medical aesthetics. The treatment should take place in a licensed healthcare facility.
Are dermal fillers safe?
Most side effects of fillers are mild and temporary. Rare complications such as a blocked blood vessel can be serious, which is why the injector's training matters. A careful examination and a clinic prepared for emergencies reduce the risk.
Can I get filler and Botox on the same day?
Yes, filler and Botox are often given in the same visit for different areas. Your doctor plans the order and amounts. Some doctors prefer to space them out when treating the same area.
