Blepharoplasty guide

What is blepharoplasty?

Blepharoplasty is the surgical removal or repositioning of excess eyelid skin, muscle and fat. It can be done on the upper lids, the lower lids or both. Upper blepharoplasty mainly treats hooded, heavy lids. Lower blepharoplasty mainly treats under-eye bags and the groove below them.

Upper lid surgery can also be functional. When skin hangs over the lashes, it can block the top of your field of vision. In that case the operation may be done for medical reasons as well as appearance. For an overview of all eye area operations, see the eyelid and eye surgery page.

How is upper and lower blepharoplasty done?

Blepharoplasty usually takes about 90 minutes when one pair of lids is treated, and longer when all four lids are done.

  1. Marking: With you sitting up, the surgeon marks the skin to be removed. For the upper lid, the line follows the natural crease.
  2. Anaesthesia: Local anaesthetic with sedation is common. General anaesthesia may be used for four-lid surgery.
  3. Upper lid: A strip of skin, sometimes with a thin band of muscle and some fat, is removed through the crease.
  4. Lower lid: Fat is removed or moved into the groove, either from inside the lid or through a cut just below the lashes. Extra skin is trimmed if needed.
  5. Support: In the lower lid, the surgeon may tighten the outer corner to keep the lid in position.
  6. Closure: Fine stitches or skin glue close the incisions.

Who is blepharoplasty suitable for?

Blepharoplasty suits adults with excess upper lid skin, lower lid bags or both, who are in good general health and have healthy eyes. Good candidates usually have:

  • Hooding that makes the lids look heavy or touches the lashes
  • Lower lid bags that persist regardless of sleep
  • Normal tear production and eyelid closure
  • Realistic expectations about a refreshed rather than changed look

A low brow can make upper lids look heavier than they are. In that case the surgeon may suggest a brow lift alone or together with blepharoplasty. Dry eye, thyroid eye disease and previous eye surgery need careful assessment first.

What is recovery like after blepharoplasty?

Most people return to social activities within about 10 days after blepharoplasty. Upper lid surgery alone often heals a little faster than lower lid surgery.

  • Days 1 to 3: Swelling and bruising peak. Cold compresses and a raised head position help.
  • Days 5 to 7: Stitches are usually removed.
  • Weeks 2 to 3: Most visible bruising has faded. Light exercise usually restarts.
  • Months 3 to 6: The scar settles and the final shape becomes clear.

Reading and screen time are fine in short sessions, but they can dry the eyes. Use lubricating drops if your surgeon recommends them.

What are the specific risks of blepharoplasty?

The specific risks of blepharoplasty relate to how much tissue is removed and how the lower lid is supported.

  • Too much skin removed from the upper lid: This can make it hard to close the eye fully.
  • Lower lid pulling down (lid retraction or ectropion): More likely with lower lid skin removal and lax lids.
  • Hollow look: Removing too much fat can make the eyes look sunken.
  • Dry eye: Often temporary but can persist in people already prone to it.
  • Asymmetry: Small differences between the sides are common, larger ones may need revision.

Rarely, bleeding behind the eye can threaten vision. Sudden severe pain, a bulging eye or loss of vision need emergency care.

How does blepharoplasty differ from other eye procedures?

Blepharoplasty treats excess skin and fat in the lids, while related procedures address the lower lid alone, the eye corner, or volume without surgery.

What affects the cost of blepharoplasty?

The cost of blepharoplasty depends mainly on whether upper, lower or all four lids are treated. Other factors include:

  • Fat removal or fat repositioning in the lower lid
  • Added procedures such as outer corner tightening or a brow lift
  • Local anaesthetic with sedation or general anaesthesia
  • The facility, the city and follow-up visits
  • Whether the surgery is partly functional

The exact fee is given after the consultation, once the surgeon has examined your lids. You can compare eyelid surgeons in the doctor list by city.

Blepharoplasty Frequently Asked Questions

Is upper or lower blepharoplasty more common?
Upper lid surgery is done more often, especially for hooded lids. Lower lid surgery is chosen for persistent bags. Many people have both at the same time.
Can blepharoplasty improve vision?
When hanging upper lid skin blocks the top of the visual field, removing it can widen what you see. This is assessed with a vision field test. Purely cosmetic surgery does not change eyesight.
Will I need to stay in hospital?
Most blepharoplasty is done as a day procedure and you go home the same day. An overnight stay may be suggested after four-lid surgery or general anaesthesia. Someone should drive you home.
How long do blepharoplasty results last?
Removed fat and skin do not come back, so results usually last many years. The upper lids may loosen again slowly with age. Lower lid results tend to last longer.
Can I wear glasses after blepharoplasty?
Glasses can usually be worn soon after surgery if they do not press on the lids. Contact lenses are postponed for longer. Ask your surgeon for your own timing.
