Under-Eye Surgery guide

What is under-eye surgery?

Under-eye surgery, or lower blepharoplasty, is an operation that treats bags and hollowing under the eyes by removing or moving lower eyelid fat. If there is extra skin, a small amount can be removed as well. The aim is a smoother line from the lower lid to the cheek.

Under-eye bags form when the fat that cushions the eye pushes forward as the tissue holding it back weakens. A groove often appears below the bag. Many surgeons now move part of this fat into the groove instead of only removing it. For an overview of eye area operations, see the eyelid and eye surgery page.

How is under-eye surgery done?

Under-eye surgery usually takes about 90 minutes and is done through one of two approaches.

  • Inside approach (transconjunctival): The cut is made on the inner surface of the lower lid. No external scar is left. It suits people with bags but little excess skin.
  • Outside approach (subciliary): The cut is made just below the lashes. It allows excess skin and muscle to be tightened as well.
  1. The surgeon assesses bags, skin quality and lower lid tone.
  2. Local anaesthetic with sedation or general anaesthesia is given.
  3. Fat is removed, or moved over the bone to fill the groove (fat repositioning).
  4. If needed, a thin strip of skin is removed and the outer corner is supported.
  5. The incisions are closed with fine stitches, or left to heal on their own in the inside approach.

Who is under-eye surgery suitable for?

Under-eye surgery suits adults whose under-eye bags come from fat bulging forward and do not improve with rest. Good candidates usually have firm enough lower lids and healthy eyes.

It may not be the right choice when:

  • Dark circles are mainly caused by pigment or visible veins
  • Puffiness changes a lot from day to day, which points to fluid retention
  • There is hollowing without bags, where filler may suit better
  • The lower lid is very loose, which needs extra support during surgery
  • You have dry eye or thyroid eye disease that is not under control

What is recovery like after under-eye surgery?

Most people return to social activities within about 10 days to two weeks after under-eye surgery. The inside approach often has a slightly faster recovery because there are no external stitches.

Swelling and bruising are strongest in the first three days. The lower lid may feel tight, and the eyes may water or feel dry. Cold compresses and sleeping with the head raised help. Avoid rubbing your eyes. The final contour usually settles over about three months.

What are the specific risks of under-eye surgery?

The specific risks of under-eye surgery relate mostly to lower lid position and to how much fat is removed.

  • Lower lid pulling down: More likely with the outside approach and loose lids. It can expose more of the white of the eye.
  • Hollow look: Removing too much fat can make the eyes look sunken.
  • Remaining bags or irregularity: Small lumps or uneven areas can need correction.
  • Dry eye and watering: Usually temporary.
  • Bleeding: Rarely, bleeding behind the eye can threaten vision and needs emergency care.

Sudden severe pain, a bulging eye or reduced vision after surgery need immediate attention.

How does under-eye surgery compare with filler and other eye procedures?

Under-eye surgery removes or moves bulging fat, while filler only adds volume to a groove. Surgery also differs from upper lid and eye corner operations in the area it treats.

OptionTreatsRecovery (usually)How long it lasts
Under-eye surgeryBags, groove, some excess skinAbout 10 to 14 daysMany years

What affects the cost of under-eye surgery?

The cost of under-eye surgery depends on the approach and how much work the lower lid needs. Factors include:

  • Inside or outside approach
  • Fat removal only, or fat repositioning
  • Skin removal and outer corner support
  • Combination with upper lid surgery
  • Type of anaesthesia, the facility and the city

The exact fee is given after examination. Look for a plastic surgeon or oculoplastic surgeon who regularly performs lower lid surgery. You can compare them in the doctor list by city.

Under-Eye Surgery Frequently Asked Questions

Does under-eye surgery leave a scar?
With the inside approach there is no visible scar. With the outside approach the scar sits just below the lashes and usually fades over months. Your surgeon will explain which approach suits you.
Will under-eye bags come back after surgery?
Removed fat does not grow back, so results usually last many years. Ageing continues, so some change can appear over time. Bags returning is less common than upper lid loosening.
Can under-eye surgery remove dark circles?
Surgery can reduce shadows caused by bags and grooves. It does not change pigment or visible veins in the skin. Other treatments may be needed for those.
Is under-eye surgery done under general anaesthesia?
It is often done under local anaesthetic with sedation. General anaesthesia may be used if other procedures are combined. The choice is made with your surgeon.
At what age is under-eye surgery done?
There is no fixed age. Some people have inherited bags in their twenties or thirties, others develop them later. Suitability depends on anatomy and health, not age alone.
