Buttock Aesthetics guide

What is buttock aesthetic surgery?

Buttock aesthetic surgery changes the volume, projection or position of the buttocks. It is one of the procedures within body contouring, which covers the general principles of shaping the body with surgery. Three main methods are used: fat transfer, buttock implants and a buttock lift. Fat transfer to the buttocks is widely known as the Brazilian butt lift, or BBL, although it does not lift sagging skin.

The choice of method depends on how much fat you have elsewhere, the quality of your skin and whether the goal is more volume or less sagging.

How is buttock surgery performed?

Each method follows a different path. In fat transfer, fat is first removed from the abdomen, flanks or thighs with liposuction. The fat is processed and injected into the buttocks with a cannula.

  1. Planning: The surgeon assesses buttock shape, skin quality and fat reserves, and marks the areas.
  2. Anaesthesia: The operation is usually done under general anaesthesia.
  3. Fat harvest or implant pocket: Fat is collected, or a pocket for the implant is created through an incision, often in the buttock crease.
  4. Placement: Fat is placed in the layer under the skin, or the implant is positioned in or under the muscle.
  5. Closure: Incisions are closed, and a compression garment is fitted.

A buttock lift removes a band of loose skin, usually along the upper edge of the buttocks, and is mostly done after major weight loss. The operation usually takes about two to three hours, longer when methods are combined.

Who is a good candidate for buttock surgery, and who is not?

Fat transfer suits people with enough spare fat and a stable weight. Implants are considered for slim people without enough fat to transfer. A lift suits people whose main concern is loose, sagging skin rather than lack of volume.

  • Very low body fat may make fat transfer impossible.
  • Smoking raises the risk of wound and fat survival problems.
  • A history of blood clots needs careful assessment.
  • Planned large weight changes can alter the result.
  • A job or lifestyle that needs long periods of sitting soon after surgery may be hard to manage.

The surgeon decides on suitability after an examination.

What is recovery like after buttock surgery?

Recovery usually takes about two to three weeks before most people return to desk work, and the final shape settles over a few months. After fat transfer, surgeons often advise avoiding sitting directly on the buttocks for a few weeks, using a special cushion when sitting is unavoidable. A compression garment is worn for several weeks. Recovery after implants may feel more painful at first, and strenuous exercise is usually postponed for several weeks.

Part of the transferred fat is absorbed by the body in the first months. The fat that survives behaves like normal fat and changes with weight gain or loss, so a second session is sometimes discussed. Implants are designed to stay long term but can need removal or replacement.

What are the risks of buttock surgery?

The most serious risk is fat embolism after fat transfer, which can be fatal. It has been linked to fat being injected into or below the buttock muscle, where large veins run. For this reason, professional societies recommend placing fat only in the layer under the skin, and some advise ultrasound guidance. Other risks include:

  • Fluid collection, infection and wound separation
  • Uneven contour, lumps or asymmetry
  • Fat necrosis, felt as firm lumps
  • Implant displacement, infection or visible edges
  • Numbness and a visible scar after a lift
  • Blood clots in the leg veins

Shortness of breath, chest pain or swelling in one leg after surgery needs emergency care.

How does buttock surgery differ from other body contouring?

Buttock surgery adds or repositions volume, while most other body contouring procedures remove fat or skin. Liposuction is often part of a fat transfer, and a tummy tuck can be done in the same plan when the abdomen also has loose skin.

What affects the cost, and how do you choose a surgeon?

The cost depends on the method, the number of liposuction areas, implant type, operating time, hospital stay, garments and the city. The exact fee is set after an examination. A specialist in plastic, reconstructive and aesthetic surgery working in a fully equipped facility should perform this surgery. Ask which layer fat will be placed in, whether ultrasound guidance is used, how blood clots will be prevented and how follow-up will work.

Buttock Aesthetics Frequently Asked Questions

How long can't I sit after a BBL?
Surgeons often advise avoiding sitting directly on the buttocks for a few weeks after fat transfer. A special cushion may be used when sitting cannot be avoided. Your surgeon will give you the exact period.
Is a Brazilian butt lift permanent?
Some of the transferred fat is absorbed in the first months. The fat that survives stays, but it changes with weight gain or loss. Some people need a second session.
Can I have a buttock implant if I am very slim?
Yes, implants are mainly considered for people without enough fat to transfer. They add volume regardless of fat reserves. Implant-specific risks such as displacement should be discussed first.
Does a buttock lift add volume?
No, a buttock lift mainly removes loose skin and raises the buttocks. It can be combined with fat transfer if more volume is wanted. It is mostly done after major weight loss.
Will my body shape change if I gain weight after fat transfer?
Yes, transferred fat behaves like normal fat. Weight gain can enlarge the buttocks and other areas, and weight loss can reduce them. A stable weight helps preserve the result.
