Permanent Lip Filler guide

What is permanent lip filler?

Permanent lip filler is not a single product or technique but a shared name for different methods meant to stay in the lips long term. They fall into three groups:

  • Fat transfer: Fat taken from an area such as the abdomen or thigh is processed and injected into the lips.
  • Permanent synthetic fillers: Materials the body does not break down, such as liquid silicone or PMMA, are injected into the lips.
  • Lip implants: A soft silicone or similar implant is placed inside the lip through small incisions.

Long-acting hyaluronic acid products are sometimes promoted as permanent filler. They are absorbed over time, so they are not permanent. General information on temporary filler is on our lip filler page.

How is permanent lip filler done?

How permanent lip filler is done depends on the method. Fat transfer is the most common and usually follows these steps:

  1. The doctor assesses your lips, the donor area for fat and your expectations.
  2. The procedure is done under local anaesthesia, with sedation if needed.
  3. A small amount of fat is removed from the abdomen or thigh with a thin cannula.
  4. The fat is washed or spun in a centrifuge to prepare it for injection.
  5. The prepared fat is placed in the lips in small amounts with a fine cannula.

Synthetic permanent fillers are injected with a needle. For a lip implant, a tunnel is made inside the lip through small incisions at the corners of the mouth and the implant is inserted. Depending on the method, the procedure usually takes 30 minutes to an hour and a half.

Who is permanent lip filler suitable for, and who is it not?

Permanent methods may be considered for people with a suitable lip structure who understand the risks and want a long-term option instead of repeated temporary filler. Most doctors suggest trying temporary filler first, so you can see whether you are comfortable with the new lip shape.

Permanent methods are usually not advised in these situations:

  • You are not yet sure about the lip shape you want
  • There is old filler of unknown content in the lips
  • Frequent cold sores, autoimmune disease or conditions that impair wound healing
  • Not enough fat tissue for transfer
  • Smoking, especially before fat transfer

Is permanent lip filler truly permanent, and what is recovery like?

How permanent it is depends on the method. Part of the transferred fat is absorbed in the first months. The part that survives stays long term, though its volume can change with your weight. Synthetic fillers and implants remain in the body, but the lips continue to change with age.

After fat transfer, the lips swell noticeably in the first days, because extra fat is placed to allow for absorption. Swelling usually eases within one to two weeks. A similar swelling phase follows an implant. The final look is assessed after a few months.

What are the risks of permanent lip filler?

The most important risk is that the material cannot easily be taken out if a problem occurs. The main risks by method are:

  • Fat transfer: Asymmetry, uneven absorption, cysts or firmness, bruising at the donor site
  • Synthetic fillers: Lumps, firmness, granulomas, infection and migration, even years later
  • Implants: Shifting, extrusion, being felt or seen, and infection
  • All methods: Blocked blood vessels, numbness and a result that falls short of expectations

Synthetic materials cannot be dissolved with hyaluronidase. Removing them needs surgery and is not always fully possible.

How does permanent lip filler differ from hyaluronic acid filler?

Permanent lip filler differs from hyaluronic acid filler in how long the effect lasts and whether it can be reversed.

MethodDurationReversibilityMain limitation
Hyaluronic acid fillerUsually 6 to 12 monthsCan be dissolved with hyaluronidaseNeeds repeating
Fat transferLong-lasting for the fat that survivesCannot be dissolvedSurvival rate is unpredictable
Synthetic permanent fillerPermanentOnly surgically, and often partlyLate granulomas
Lip implantPermanentCan be removed surgicallyShifting and being felt

What affects the cost of permanent lip filler, and how do you choose a doctor?

The cost depends on the method, the type of anaesthesia and the number of sessions. The main factors are:

  • Choice of fat transfer, synthetic filler or implant
  • Local anaesthesia, sedation or an operating theatre
  • Extra sessions for fat transfer
  • The type of implant used
  • The clinic's city

The final fee is set after a consultation. Fat transfer and implants are surgical procedures, usually done by plastic surgeons. Ask for the name and documentation of the material, how it would be removed if a problem arises and whether further sessions may be needed. You can compare doctors who perform permanent lip procedures by city in the doctor listings.

The information on this page is for general information only and is not medical advice. The suitable method for you can only be determined by a physician examination.

Permanent Lip Filler Frequently Asked Questions

Does long-lasting hyaluronic acid count as permanent filler?
No. Even the longest-lasting hyaluronic acid products are absorbed over time. Some are marketed as permanent, but their effect lasts only for a limited period. Ask your doctor about the product and how long it is expected to last.
Is liquid silicone used in the lips?
Liquid silicone injected into the lips is linked to firmness, lumps and granulomas that can appear years later. In many countries it is not approved for this use. Removing it often needs surgery and can leave scars.
Can permanent lip filler be removed later?
Fat that has survived after transfer is permanent and cannot be dissolved. Synthetic fillers and implants can only be removed surgically. Because synthetic fillers spread into tissue, complete removal is not always possible.
Can hyaluronic acid be added over old permanent filler?
Adding new filler over a permanent or unknown old filler can raise the risk of problems. The doctor first assesses the type and condition of the old material. An ultrasound scan may be used.
How much bigger will my lips get with fat transfer?
How much of the transferred fat survives cannot be known in advance. Less volume than expected may remain after the first session. More than one session may be needed to reach the desired result.
