Nasal Tip Plasty guide

What is nasal tip plasty?

Nasal tip plasty is an operation that reshapes the cartilage in the lower third of the nose. It is also called tip rhinoplasty. The bridge and bony framework are usually left untouched.

It is used for concerns such as:

  • A wide, round or bulbous tip
  • A drooping or overly upturned tip
  • An asymmetric tip or a tip that looks split in two
  • Wide or irregular nostrils

If there is a hump, a crooked bridge or a wide nasal bone, tip plasty on its own is not enough and full rhinoplasty is planned instead. For general information on nose surgery, see our rhinoplasty page.

How is nasal tip plasty performed?

Tip plasty reaches the lower lateral (alar) cartilages through an open or closed approach and usually takes one to two hours.

  1. Consultation: The surgeon assesses tip support, skin thickness and the angle between the nose and upper lip.
  2. Anaesthesia: Most procedures are done under general anaesthesia. Local anaesthesia with sedation may be chosen for limited work.
  3. Access: The cartilage is exposed through incisions inside the nose or a small incision across the columella.
  4. Reshaping: Small pieces of cartilage are removed, the cartilage is reshaped with stitches, or a support graft is added.
  5. Closure: Incisions are closed and the tip is supported with tape or a small splint.

Lifting a drooping tip often needs extra support. This usually comes from cartilage taken from the septum, the wall between the nostrils.

Who is nasal tip plasty suitable for, and who is it not?

Tip plasty suits people who are happy with the bridge of their nose and only want to change the shape of the tip. The surgeon decides on suitability after an examination.

Results may be limited, or a different plan may be needed, in these situations:

  • A clear hump or crooked bridge
  • Very thick skin over the tip
  • Breathing problems from a deviated septum
  • Previous nasal surgery
  • A nose that is still growing

With thick skin, fine changes in the cartilage are less visible from the outside. The surgeon should explain this limit clearly before surgery.

How long is recovery after tip plasty, and is the result permanent?

Most people return to daily activities within 7 to 10 days. Because the bones are not touched, bruising around the eyes is often milder than after full rhinoplasty, or absent.

The tip is the slowest part of the nose to settle. Swelling there eases over months, and fine detail can take up to a year to appear. With thick skin it may take longer. In the first weeks, avoid blowing your nose, pressing on the tip and heavy exercise.

Changes to the cartilage are permanent. The tip may drop slightly with age, so surgeons usually aim to build lasting support.

What are the risks of nasal tip plasty?

The main risks of tip plasty are prolonged swelling, asymmetry and the tip dropping over time. Risks specific to the tip include:

  • The tip ending up too upturned, or dropping later
  • Cartilage edges becoming visible under the skin
  • Nostril asymmetry or collapse of the nostril rim
  • Long-lasting numbness of the tip
  • A scar on the columella after the open approach
  • Fullness at the tip from scar tissue in thick skin

Removing too much cartilage can cause the nostrils to collapse inwards when breathing in. Contact your surgeon at once if you have sudden pain, fever or a colour change at the tip.

How does nasal tip plasty differ from full rhinoplasty?

Tip plasty treats only the cartilage at the tip, while full rhinoplasty addresses the whole structure of the nose, including bone. The table compares them with revision surgery.

What affects the cost of tip plasty, and how do you choose a surgeon?

The cost of tip plasty depends on the technique and whether grafts are needed. The main factors are:

  • Open or closed approach
  • Whether septal or ear cartilage grafts are needed
  • Type of anaesthesia and length of surgery
  • The type of facility and its city

The final fee is set after a consultation. In Turkey, tip plasty is performed by plastic surgeons and ENT specialists. Ask why the surgeon suggests treating only the tip, how your skin thickness may affect the result and who will carry out the follow-up visits. You can compare doctors who perform nose surgery by city in the doctor listings.

Nasal Tip Plasty Frequently Asked Questions

Will I need a cast or splint after tip plasty?
Because the bones are not touched, a bone splint is often not needed. The tip may be supported with tape or a small splint. Your surgeon decides how long it stays on.
Can tip plasty be done under local anaesthesia?
Some limited procedures can be done under local anaesthesia with sedation. Many surgeons still prefer general anaesthesia. The type is decided after the consultation.
Can a drooping nasal tip be lifted without surgery?
Fillers or botulinum toxin can make a limited, temporary change at the tip. They do not change the cartilage structure. Filler in the nose also carries a risk of blocking a blood vessel.
Will my tip drop when I smile after surgery?
In some people, a lip muscle pulls the tip down when they smile. The surgeon assesses this at the consultation. If needed, a step addressing this muscle can be added to the plan.
At what age can tip plasty be done?
Tip plasty is done once the nose has finished growing, usually in the late teens. Anyone under 18 needs consent from a parent or guardian.
