Revision Rhinoplasty guide

What is revision rhinoplasty?

Revision rhinoplasty is an operation to correct problems that remain after previous rhinoplasty or septoplasty. It is also called secondary rhinoplasty. The problem may concern appearance, breathing or both.

Common reasons for revision include:

  • A hump that remains or forms again on the bridge
  • An over-reduced bridge, creating a scooped or saddle nose
  • An asymmetric, drooping or overly upturned tip
  • Nostril rims that collapse inwards
  • Breathing difficulty that began or worsened after surgery
  • A hole in the septum

A revision is more complex than a first operation. General information on first-time surgery is on our rhinoplasty page.

How is revision rhinoplasty performed?

Revision rhinoplasty works through scar tissue to assess the remaining structures and rebuild missing support with grafts. It usually takes two and a half to four hours.

  1. Waiting: A gap of at least a year after the first operation is usual, so the tissues can mature.
  2. Reviewing records: The previous operation note is requested if possible. Nasal endoscopy or a CT scan may be done.
  3. Graft planning: If too little septal cartilage remains, ear or rib cartilage is used. The source is discussed at the consultation.
  4. Surgery: Done under general anaesthesia, usually with the open approach. The surgeon separates scar tissue, restores missing support and corrects irregularities.
  5. Closure and splint: Incisions are closed and the nose is supported with a splint and, if needed, internal splints.

It is not possible to see exactly what the earlier surgery left behind before the operation, so the surgeon may make small changes to the plan during surgery.

Who is revision rhinoplasty suitable for, and who is it not?

Revision suits people who have fully healed from earlier surgery, whose problem can be clearly defined and who are in good general health. The surgeon decides on suitability after an examination.

Revision is postponed or assessed carefully in these situations:

  • Not enough time has passed since the first operation
  • Swelling has not yet settled
  • The nasal skin is very thin or has a poor blood supply
  • Smoking
  • Excessive concern about the previous result or signs of body dysmorphic disorder

Some small irregularities become less noticeable over time. In such cases, the surgeon may recommend waiting instead of surgery.

How long is recovery after revision rhinoplasty?

Most people return to daily activities within two to three weeks. The splint usually comes off after about a week. Swelling and bruising may last longer than after the first operation.

Because of scar tissue, the final shape may take more than a year to settle. If rib cartilage was used, the chest may be sore for several weeks. Contact sports and glasses resting on the nose should wait for the time your surgeon advises. Changes made to cartilage and bone are permanent.

What are the risks of revision rhinoplasty?

The risks are higher than in a first operation because of scar tissue and reduced blood supply. The main risks specific to revision are:

  • Prolonged swelling and firmness at the tip
  • Poor blood supply to the skin
  • A graft shifting, warping or being absorbed
  • Pain and a scar at the rib donor site and, rarely, injury to the lining of the lung
  • Breathing difficulty that continues
  • The need for a further correction

Contact your surgeon at once if the nasal skin changes colour, pain increases or you develop a fever.

How does revision rhinoplasty differ from first-time rhinoplasty?

Revision rhinoplasty is a longer and less predictable operation than first-time rhinoplasty because of scar tissue and missing cartilage.

What affects the cost of revision rhinoplasty, and how do you choose a surgeon?

The cost depends on the scope of the problem and the grafts needed. The main factors are:

  • The number and difficulty of problems to correct
  • Whether rib or ear cartilage is needed
  • Length of surgery and the anaesthesia team
  • Hospital stay and follow-up visits
  • The city where the facility is located

The final fee is set after a consultation. In Turkey, revision rhinoplasty is performed by plastic surgeons and ENT specialists. Ask about the surgeon's revision experience, why they chose a particular graft source and what happens if a further correction is needed. You can compare doctors who perform revision nose surgery by city in the doctor listings.

Revision Rhinoplasty Frequently Asked Questions

How long should I wait before revision rhinoplasty?
A revision is usually planned at least a year after the first operation so the tissues can heal. With thick skin the wait may be longer. If a serious airway blockage is present, the surgeon may consider earlier surgery.
Should the original surgeon perform my revision?
This is a personal choice. The original surgeon knows how the nose looked before, while another surgeon may bring a fresh view. What matters most is seeing a specialist with revision experience.
Does taking rib cartilage leave a scar?
Rib cartilage is taken through a small incision under the chest and leaves a thin scar. In women the incision is often placed in the breast crease. The area can be painful for the first days.
Do I need surgery again for a small bump?
Small irregularities can sometimes be corrected with a limited procedure. Some doctors may suggest non-surgical options such as filler, but filler in a previously operated nose is considered to carry a higher risk of blocking a blood vessel. Options are decided after an examination.
Might I need another operation after a revision?
Yes, some people need a further correction. Scar tissue increases with each operation and makes tissues less predictable. Your surgeon should explain this possibility beforehand.
