Breast Augmentation guide

What is breast augmentation?

Breast augmentation increases the size of the breasts or restores volume lost after pregnancy or weight loss. It is most often done with implants, and sometimes with fat transfer. It is one of the procedures within breast aesthetics, where the general principles of breast surgery are explained.

Implants have an outer silicone shell and are filled with either silicone gel or saline. They come in round and teardrop (anatomical) shapes and with smooth or textured surfaces. Many surgeons now prefer smooth implants because textured implants have been linked to a rare lymphoma called BIA-ALCL.

How is breast augmentation performed?

Breast augmentation is done under general anaesthesia and usually takes one to two hours.

  1. Sizing: Chest measurements, breast tissue thickness and skin quality guide implant size and shape. Trial sizers may be used.
  2. Incision: The implant is inserted through the fold under the breast, the edge of the areola or the armpit.
  3. Pocket: A space is made in front of the chest muscle, behind it, or partly behind it (dual plane).
  4. Placement: The implant is inserted and its position is checked with you sitting up.
  5. Closure: Incisions are closed and a supportive bra is fitted.

Placement behind the muscle can give more cover in slim people, while placement in front may mean an easier recovery. Fat transfer uses liposuction to collect your own fat and gives a more modest increase.

Who is breast augmentation suitable for, and who is it not?

It suits adults whose breasts have finished developing, who are in good health and who want more volume or a better balance between the breasts.

  • Pregnancy or breastfeeding
  • An uninvestigated breast lump or abnormal imaging
  • Significant sagging, which may need a lift as well
  • Active infection
  • Smoking
  • Expectations of an implant lasting a lifetime

These situations need assessment first. The surgeon decides on suitability after an examination.

What is recovery like, and how long do implants last?

Most people return to desk work within about one week, and recovery usually takes around two weeks. Chest tightness and soreness are common at first, especially when the implant is behind the muscle. A supportive bra is worn day and night for several weeks. Heavy lifting and upper body exercise are usually postponed for about four to six weeks. Implants can sit high at first and settle over a few months.

Implants are not lifetime devices. They may need replacing because of rupture, hardening or changes in breast shape with age. Silicone implant rupture can be silent, so imaging is advised at intervals your surgeon recommends.

What are the risks of breast augmentation?

The specific risks of augmentation include:

  • Hardening of the scar tissue around the implant (capsular contracture)
  • Implant rupture, leakage or displacement
  • Rippling or visible implant edges, especially in slim people
  • Changes in nipple and breast sensation
  • Asymmetry and visible scars
  • Infection, bleeding or haematoma
  • BIA-ALCL, a rare lymphoma linked mainly to textured implants
  • Systemic symptoms reported by some people, often called breast implant illness, which are still being studied

New swelling, a lump or a change in shape years later should be checked. Always tell the radiology team you have implants before a mammogram.

How does augmentation differ from other breast surgery?

Augmentation adds volume. A breast lift raises sagging tissue, breast reduction removes tissue, and gynecomastia surgery treats breast enlargement in men.

OptionEffectRecovery for desk workPoint to consider
Implant augmentationClear increase in sizeAbout one weekImplants may need replacing
Fat transfer augmentationModest increaseAbout one weekPart of the fat is absorbed
Augmentation with liftVolume and liftAbout one to two weeksMore scarring
Breast lift aloneLift without added volumeAbout one to two weeksDoes not add upper fullness

What affects the cost, and how do you choose a surgeon?

The cost depends on the implant brand and type, whether a lift or fat transfer is added, anaesthesia, hospital stay, imaging, bras and the city. The exact fee is set after an examination. Choose a specialist in plastic, reconstructive and aesthetic surgery, and ask about the implant brand, surface and documents, whether you will receive an implant card, where the incision and pocket will be, and what happens if replacement is needed later.

The information on this page is for general information only and is not medical advice. The suitable method for you can only be determined by a physician examination.

Breast Augmentation Frequently Asked Questions

Silicone or saline implants: which is better?
Neither is better for everyone. Silicone gel tends to feel more like breast tissue, while saline implants deflate visibly if they rupture. Your surgeon will discuss which suits your body and preferences.
Can I breastfeed with breast implants?
Many people can breastfeed after augmentation. The incision site and implant position can affect it, with incisions around the areola carrying more risk. Mention any future breastfeeding plans at your consultation.
What size implant should I choose?
Size depends on your chest width, breast tissue and skin, not only on the look you want. Implants that are too large for your frame raise the risk of complications. Trial sizers can help you decide with your surgeon.
Do implants need to be changed every ten years?
There is no fixed replacement interval. Implants are replaced when a problem such as rupture or hardening occurs, or if you want a change. Regular check-ups and imaging help detect problems.
What is the difference between over and under the muscle?
Implants under the muscle get more soft tissue cover, which can look more natural in slim people. Implants over the muscle may mean less pain early on. Your surgeon recommends a position based on your tissue thickness.
