Breast Reduction guide

What is breast reduction?

Breast reduction, medically called reduction mammoplasty, removes part of the breast tissue, fat and skin to make the breasts smaller, lighter and higher. It is one of the procedures within breast aesthetics, where the general principles of breast surgery are explained.

Unlike most cosmetic breast surgery, reduction often has a medical purpose. Very large breasts can cause neck, back and shoulder pain, grooves from bra straps, rashes under the breast and difficulty exercising. Reducing the breasts aims to relieve these symptoms as well as improve proportions.

How is breast reduction performed?

Breast reduction is done under general anaesthesia and usually takes about two to three hours.

  1. Assessment: Breast size, sagging, skin quality and symptoms are assessed. Breast imaging may be requested depending on age and risk.
  2. Marking: The new nipple position and incision lines are drawn while you stand.
  3. Incision: The pattern depends on the amount of tissue to remove.
  4. Reduction: Excess tissue is removed, the nipple and areola are moved higher on a pedicle that keeps their blood supply, and the breast is reshaped.
  5. Closure: Incisions are closed, drains may be placed and a supportive bra is fitted.

Common incision patterns are around the areola and straight down (vertical or lollipop pattern), or with an extra cut in the breast fold (anchor or inverted-T pattern) for larger reductions. Liposuction alone is sometimes used for mild cases with mainly fatty breasts.

Who is breast reduction suitable for, and who is it not?

It suits adults with large breasts causing physical symptoms or discomfort, whose breasts have stopped growing and whose weight is stable.

  • Pregnancy or breastfeeding
  • Plans for pregnancy soon, since breasts may change again
  • A strong wish to breastfeed in the future
  • An uninvestigated breast lump
  • Smoking, which raises the risk of wound healing problems
  • Significant planned weight loss

These situations need assessment first. The surgeon decides on suitability after an examination.

What is recovery like after breast reduction?

Most people return to desk work in about two weeks, and recovery usually takes around three weeks. Soreness, swelling and tightness are common at first. A supportive bra is worn day and night for several weeks. Lifting and strenuous exercise are usually limited for about four to six weeks. Scars are red at first and fade over many months, but they remain visible.

The result is long lasting, although pregnancy, weight change and ageing can alter breast size and shape later. Many people notice relief from neck and back pain soon after recovery.

What are the risks of breast reduction?

The specific risks include:

  • Reduced ability to breastfeed
  • Changes in nipple sensation, sometimes permanent
  • Poor blood supply to the nipple, rarely leading to partial or full loss
  • Wound separation, often where the incisions meet
  • Thick or widened scars
  • Asymmetry in size, shape or nipple position
  • Fat necrosis, felt as firm lumps

Removed tissue is usually sent for pathology examination.

How does breast reduction differ from other breast surgery?

Reduction removes volume, while breast augmentation adds it and a breast lift raises the breast without making it much smaller. In men, enlarged breasts are treated with gynecomastia surgery.

Breast liftLittle or noneRaise sagging breastsSimilar scars
Breast augmentationMore volumeLarger breastsImplants may need replacing

What affects the cost, and how do you choose a surgeon?

The cost depends on the amount of tissue removed, operating time, hospital stay, pathology, imaging, bras and the city. The exact fee is set after an examination. Choose a specialist in plastic, reconstructive and aesthetic surgery, and ask which incision pattern is planned, how nipple blood supply will be preserved, what the effect on breastfeeding may be and how scars will be managed.

The information on this page is for general information only and is not medical advice. The suitable method for you can only be determined by a physician examination.

Breast Reduction Frequently Asked Questions

Can I breastfeed after breast reduction?
Some people can breastfeed after reduction, but the ability may be reduced. Techniques that keep the nipple attached to the underlying tissue help preserve it. Discuss your plans with your surgeon before surgery.
How much smaller will my breasts be after reduction?
The amount depends on your starting size, your goals and what keeps the breast safe and in proportion. The surgeon explains the expected change at the consultation. Cup sizes vary between brands, so they are only a rough guide.
Will breast reduction relieve back pain?
Many people with very large breasts report less neck, back and shoulder pain after reduction. Not all pain is caused by breast size, so results vary. Your doctor will assess other causes as well.
Are breast reduction scars permanent?
Yes, the scars remain, but they usually fade and flatten over many months. Their shape depends on the incision pattern. Scar care advice from your surgeon can help.
Can breast reduction be done with liposuction only?
Liposuction alone can work for mild reductions when the breast is mostly fat. It does not lift the breast or remove skin. Most people need the standard surgical technique.
