Gum Aesthetics guide

What is gum aesthetics?

Gum aesthetics covers surgical procedures that make the gums look in harmony with the teeth. It is also called pink aesthetics. An uneven gum line can affect a smile even when the teeth themselves are well shaped.

Gum aesthetics is part of cosmetic dentistry. It can be done on its own or as the first stage of a smile design plan. Adjusting the gum level before crowns or veneers are placed is common.

Gum aesthetics addresses three main problems:

  • A gummy smile, where a large amount of gum shows when smiling.
  • Gum recession, where the gum pulls back and exposes the tooth root.
  • Colour differences, meaning dark patches (pigmentation) on the gums.

What causes a gummy smile?

A gummy smile has more than one possible cause, and the right treatment depends on the cause. In some people, the gum does not move back to its usual level after the teeth erupt, so the teeth look short. Gum surgery can be a good option here. Some medicines and gum inflammation can also make the gums overgrow.

The cause is not always the gum. An upper lip that lifts too high or excess vertical growth of the upper jaw can create the same look. In these cases gum surgery alone is not enough, and the dentist may suggest other treatments for lip or jaw causes.

How is gum aesthetics done?

Gum aesthetics is done under local anaesthesia, usually in a single session. For excess gum, the procedure generally follows these steps:

  1. The dentist examines gum health, tooth length and the smile line.
  2. X-rays are used to check the bone level under the gum.
  3. The new gum line is planned, sometimes with a digital design or a temporary mock-up.
  4. The area is numbed with local anaesthesia.
  5. Excess gum is removed with a scalpel or laser and the edges are shaped.
  6. If the bone level also needs adjusting, crown lengthening is performed.
  7. Care and eating advice is given and a follow-up visit is booked.

For gum recession, a graft is usually used. A small piece of tissue from the palate is placed over the exposed area. In some cases the nearby gum is moved to cover the root surface.

Who is gum aesthetics suitable for, and who is it not?

Gum aesthetics suits people with healthy gums whose problem comes from the gum itself. The dentist decides on suitability after an examination. Common reasons for treatment include:

  • Too much gum showing when smiling
  • Gum levels that differ between the left and right sides
  • Front teeth that look short
  • Exposed, sensitive tooth roots
  • Dark patches on the gums

Active gum inflammation is treated first. Uncontrolled diabetes and smoking can slow healing. If you take blood thinners, tell your dentist. If the problem comes from the lip or jaw, gum surgery alone may not be enough.

How long does healing take, and are the results lasting?

The procedure usually takes 30 minutes to an hour and a half, depending on the number of teeth and whether bone work is needed. After simple gum removal, surface healing generally takes one to two weeks. Crown lengthening or grafts can extend healing to several weeks, and the gum line can take a few months to settle. If crowns are planned, the dentist may wait for this.

In the first days, avoid brushing the treated area on day one, then clean it gently with a soft brush. Use any mouthwash your dentist recommends, do not smoke and do not touch the area with your tongue or fingers. Contact your dentist if bleeding does not stop, swelling increases or you develop a fever.

Well-planned gum changes are long-lasting. If the bone level is ignored, the gum can grow back over time. Removed dark patches may also return in some people.

What are the risks and side effects of gum aesthetics?

Gum aesthetics is surgery and carries some risks. Possible side effects include:

  • Pain, swelling and mild bleeding in the first days
  • Pain for a few days at the palate where a graft is taken
  • Sensitivity in teeth with exposed root surfaces
  • An uneven gum line
  • Regrowth of gum tissue
  • Small triangular gaps between the teeth
  • Graft failure
  • Infection

Crown lengthening also involves bone, so healing takes longer. Your dentist should explain which risks are more likely in your case.

How do the gum aesthetics methods differ?

Gum aesthetics methods differ by the problem they treat. The table gives a general comparison.

MethodUsed forWhat is done
Gingivectomy and gingivoplastyExcess or uneven gumExcess gum is removed and reshaped
Crown lengtheningShort teeth where bone level also needs adjustingGum and bone are reshaped together
Gum graftGum recessionTissue from the palate is placed over the receded area
DepigmentationDark patches on the gumsThe pigmented surface layer is removed with a laser or scalpel

What determines the cost of gum aesthetics?

The cost depends on the method used and the number of teeth treated. The price range on this page gives only a general idea. The main factors are:

  • Number of teeth or areas treated
  • Whether bone work is needed
  • Whether a graft is used, and which type
  • Use of a scalpel or laser
  • Any gum treatment needed beforehand
  • The clinic's city and the dentist's experience

The final fee is set after an examination.

What should you look for when choosing a dentist for gum aesthetics?

Experience in gum surgery matters most. The specialty for this area is periodontology, although experienced general dentists also carry out simple gum contouring. When choosing, you can ask:

  • Was the cause of the excess gum investigated at the examination?
  • Was the bone level checked with X-rays?
  • Were you shown the planned new gum line?
  • Were later stages, such as crowns or veneers, planned if needed?
  • Were healing and risks explained clearly?

You can compare periodontists and dentists by city in the doctor listings on this page.

The information on this page is for general information only and is not medical advice. The suitable method for you can only be determined by a physician examination.

Gum Aesthetics Frequently Asked Questions

Is gum contouring painful?
The procedure is done under local anaesthesia. Once it wears off, pain and tenderness may last a few days. The palate area where a graft is taken can be more uncomfortable for some people.
When can I eat normally after gum surgery?
Soft, lukewarm foods are advised for the first days. Hard, spicy and very hot foods are usually avoided for a while. Your dentist will tell you when to return to a normal diet, based on the procedure.
Can gums grow back after gum contouring?
When the procedure takes the bone level into account, the gum usually keeps its new position. If bone work was needed but only gum was removed, the tissue can grow back over time. This is why bone level is checked on X-rays during planning.
Is laser gum contouring different from a scalpel?
A laser can reduce bleeding during cutting. A scalpel is widely used for shaping the gum edge. The tool is chosen based on the procedure and the dentist's experience.
Can gum recession be treated?
Gum recession can be covered with methods such as grafts or moving the gum tissue. How much can be covered depends on the degree of recession and the bone level between the teeth. Causes such as hard brushing also need to be corrected.
