Dental Implant guide

What is a dental implant?

A dental implant is a screw-shaped artificial root placed in the jawbone in place of a lost tooth's root. It is usually made of titanium or a titanium alloy. Zirconia implants are used less often.

Implant treatment has three main parts:

  • Implant body: the screw placed inside the bone.
  • Abutment: the connector that links the implant to the tooth above it.
  • Restoration: the crown, bridge or denture you see in the mouth.

The materials used for the restoration are similar to those used for ordinary crowns, and zirconia is a common choice. Because it restores a missing tooth, implant treatment also falls within dental aesthetics.

How does an implant fuse with the bone, and why does bone volume matter?

Osseointegration is the direct bonding of the implant surface to the surrounding bone. The implant can carry chewing forces only once this bond has formed. It usually takes 2 to 6 months. The lower jaw has denser bone, so healing there is often faster than in the upper jaw. Bone grafting or a sinus lift can extend the waiting time. In selected cases with good bone, a temporary tooth can be fitted soon after placement, depending on how stable the implant is at the start.

Bone volume means having enough bone height and width for the implant to sit firmly. After a tooth is lost, the jawbone gradually shrinks, so areas that have been without teeth for a long time may lack bone. Bone is usually measured with cone beam computed tomography (CBCT), which also shows the position of nerves and the sinus. If bone is lacking, these options may be considered:

  • Bone graft: missing bone is rebuilt with graft material or a bone block.
  • Sinus lift: in the back of the upper jaw, the sinus floor is raised and graft is placed beneath it.
  • Short or narrow implants: in suitable cases these can reduce the need for grafting.

How is a dental implant placed?

Implant treatment has planning, surgical placement, healing and restoration stages. The process usually follows these steps:

  1. An examination is done, and your general health and medications are reviewed.
  2. X-rays and a CT scan assess bone volume.
  3. Decay, gum disease or extractions are treated first if needed.
  4. The implant is placed in the bone under local anaesthesia. A single implant usually takes about 30 minutes to an hour.
  5. A graft is placed at the same visit or in a separate procedure if needed.
  6. A healing period of a few months follows, during which a temporary tooth can be worn.
  7. After healing, impressions are taken and the abutment and crown are made.
  8. The crown or denture is fitted and the bite is checked.

Who is a good candidate for a dental implant, and who is not?

Implants suit people whose jaw growth is complete, whose general health allows minor surgery and who have enough bone or can gain it with grafting. The dentist decides on suitability after an examination and imaging.

Some factors can reduce the success of an implant:

  • Smoking can impair bone healing and gum healing.
  • Uncontrolled diabetes increases the risk of infection and failed fusion.
  • Past or untreated gum disease raises the risk of inflammation around the implant.
  • Poor oral hygiene can lead to implant loss over time.
  • Severe teeth grinding puts extra load on the implant and crown.
  • Radiotherapy to the head and neck can make bone healing harder.
  • Bone medicines such as bisphosphonates or denosumab need assessment for jawbone problems.

These factors do not always rule out an implant. Tell your dentist about every medicine you take.

How long does implant treatment take, and how long does it last?

The whole treatment usually takes 3 to 6 months, and longer if grafting is needed. Pain, swelling and mild bruising are common in the first days after surgery and usually ease within a few days to a week. Most people return to daily activities within a day or two. Soft foods are advised at first.

A well-integrated implant can last many years with good care. Implants do not decay, but the gum and bone around them can become diseased. The crown on top may need replacing over time. Regular check-ups are the basis of long-term success.

What are the risks and side effects of dental implants?

An implant is a surgical procedure and carries some risks. Possible complications include:

  • Pain, swelling, bruising and bleeding
  • Infection at the surgical site
  • Failure of the implant to fuse with the bone, requiring removal
  • Numbness of the lip or chin when working near the nerve in the lower jaw
  • A tear in the sinus membrane in the upper jaw
  • Inflammation around the implant (peri-implantitis) and bone loss
  • A loose screw or a broken crown

Numbness from nerve injury is often temporary but can be permanent in some cases. Planning with CT helps reduce these risks. An implant that fails to fuse can often be placed again after the area heals.

What is the difference between an implant, a bridge and a denture?

Implants, bridges and removable dentures are the three main ways to replace missing teeth. The table gives a general comparison.

What affects the cost, and how do you choose a dentist?

The cost depends on the number of implants, the bone situation and the type of restoration. Other factors include the implant brand and system, the need for grafting or a sinus lift, CT scans and surgical guides, the restoration material, the city and the dentist's experience. The price range on this page gives only a general idea; the exact fee comes from the treatment plan.

Implants are placed by dentists trained in implant surgery, and oral and maxillofacial surgeons and periodontists also work in this field. Prosthodontists may handle the restoration stage. When choosing, you can check:

  • Is a CT scan taken before treatment?
  • Are the brand and documents of the implant shared with you?
  • Are the need for grafting and the total treatment time explained clearly?
  • Are your risk factors and medications reviewed?
  • Is a long-term check-up schedule planned?

You can compare dentists listed on DoktorNo by city.

Dental Implant Frequently Asked Questions

Is getting a dental implant painful?
Implants are placed under local anaesthesia, so pain is usually not felt during the procedure. Pain and swelling can last a few days once the anaesthetic wears off. These are usually managed with the pain relief your dentist recommends.
Can smokers get dental implants?
Smoking does not always prevent an implant, but it raises the risk of failed fusion and inflammation around the implant. Dentists advise stopping before treatment and during healing. The risk may depend on how much you smoke.
Can people with diabetes get implants?
Implants can be placed in people whose blood sugar is under control. Uncontrolled diabetes raises the risk of infection and failed fusion. Your dentist may review your blood sugar results before treatment.
Can you get an implant if you do not have enough bone?
Many people with too little bone can still have implants after a bone graft or sinus lift. These procedures make treatment longer. The right method is chosen after bone volume is measured on a CT scan.
Can an implant be placed the same day a tooth is removed?
In some cases an implant can be placed right after extraction, if there is no infection and the bone is stable. Otherwise the socket is left to heal first. Your dentist decides after examining the area and the imaging.
